We hope you find our website helpful. It tells you about the surgery, the services that we offer and provides links to more information and support. The main surgery is at Madeley and there is a branch surgery at Baldwins Gate.

Appointments and prescriptions may be made using the buttons on the right of the screen. Some forms, such as information for holiday travel, may be downloaded. Use the search box to look for particular items of interest.

Let us know if there is any other information that you think would be useful to put on the website.

The branch surgery re-opened on Tuesday 20th April following significant building work and refurbishment. As yet we do not have a date for the reinstallation of our NHS computer system or of a telephone system. Until we have a date we will continue to provide a once weekly service. This will be at Baldwins Gate Surgery every Tuesday morning between 9am and 12 noon.
BALDWINS GATE OPEN EVENING: Thursday 6th May 2010 between 5.00 pm and 7.00 pm.

In reference to Human Swine Flu, if you have flu-like symptoms and are concerned you may have swine flu, may we kindly ask that you do not come to the surgery, but telephone us for advice on 01782 750274.
