We hope you find our website helpful. It tells you about the surgery, the services that we offer and provides links to more information and support. The main surgery is at Madeley and there is a branch surgery at Baldwins Gate. The branch surgery is currently closed for building work (see News).

Appointments and prescriptions may be made using the buttons on the right of the screen. Some forms, such as information for holiday travel, may be downloaded. Use the search box to look for particular items of interest.

In reference to the Human Swine Flu outbreak. If you have flu-like symptoms and are concerned you may have swine flu, may we kindly ask that you do not come to the surgery, but telephone us for advice on 01782 750274.
