We hope you find our website helpful. It tells you about the surgery, the services that we offer and provides links to more information and support. The main surgery is at Madeley and there is a branch surgery at Baldwins Gate.
Appointments and prescriptions may be made using the buttons on the right of the screen. Some forms, such as information for holiday travel, may be downloaded. Use the search box to look for particular items of interest.
Let us know if there is any other information that you think would be useful to put on the website.
The menopause, HRT, osteoporosis and all that ... An evening talk with local gynaecologists. Miss O'Mahoney and Miss Indusekhar will talk on these issues, with plenty of time for questions. The meeting is free, you don't need to be a patient at Madeley Practice, and you may bring friends along. No need to book. If you would like to be involved in the practice survey to improve services in your surgery, please ask for a form at reception so we can contact you by email (or post if you are not online).
There will be an initial survey to determine the most important areas you want us to look at.
Following that there will be the full survey which should take around 5 minutes, depending upon comments you wish to add.
Results will be sent out online and published on the website and in the newsletter.
We need as many of our patients as possible, if you do not have computer facilities we can arrange paper copies for you to collect.
If you would like to be involved, please email us and we will add you to our email list.
We will then contact you to invite you to participate in the surveys we do for the Practice.
