Audit


You should record at least six significant event analyses(SEA) during each ST year and a detailed audit showing completion of a second cycle during ST2/ST3. We also expect you to carry out at least one mini-audit at the Practice during ST3.

Your audit needs to be uploaded into your e-portfolio before the ARCP panel at the end of ST3.

Examples of recent audits at the practice include:

  • appropriateness of 14 day cancer clinic referrals
  • adverse drug reactions in the elderly
  • use of b blockers in heart failure
  • patient DNAs
  • use of tiotropium and long acting B2 agonists in COPD


Significant Event Analysis

Examples of recent significant events at the Practice include:

  • breach of confidentiality with telephoned test result
  • chemist prescription of inappropriate flu vaccine for four year old child
  • patient death from Guillain Barre after routine flu vaccination
  • inappropriate management of an INR result
  • computer coding issue on smoking statue on a patient record printout


Primary Care Trigger Tool

The Primary Care Trigger Tool was developed by the NHS Insitute, and uses rapid structured casenote reviews. It takes about five minutes per patient to acrry out the review. a quick first pass screen is carried outlooking for triggers i.e features in the notes suggesting vulnerable patients. Any patient with a trigger present in the last year is reviewed in more detail to consider whether there were any adverse events or near misses relating to the trigger. Those identified can then be reviewed as an SEA. Repeating the PCTT should show a reducing rate of events.
