Baldwins Gate Surgery: Closure for re-development

DETAILS OF ALTERNATIVE ARRANGEMENTS

Building work starts on Monday 2nd November and should be completed by March 2010. The surgery will be closed throughout this period.
We apologise for the inconvenience that this will cause but the refurbished and extended building with off road parking will provide you with an up to date surgery that remains in your village.

Alternative arrangements:
• Every TUESDAY from 9.00 am to 12 noon prescriptions can be collected from Whitmore Village Hall

• Blood tests by appointment will change to Tuesday morning at Whitmore Village Hall

• Appointments to see a doctor or nurse will only be available at Madeley. There will be no appointments at the Village Hall.
Our receptionists at Madeley can help you with any requests – do explain to them any problems that you may have and we will do our best to be flexible and help you. You can also contact Julie Lawton, Practice Manager, regarding any problems that may arise.

Monday 8.30 am – 8.15 pm (open between 6.00 – 8.15 pm for pre-booked appointments only - phone lines open until 6.00pm)
Tuesday 8.30 am – 6.00 pm

Thursday 8.30 am – 1.00 pm
Friday 8.30 am – 6.00 pm
