The NHS in England is introducing an electronic care record. You will have received information in the post about the service. for more information contact the NHS Care Records Line on 0845 603 8510 or visit www.nhscarerecords.nhs.uk/

If you do not want to have a Summary Care Record on the national system you must complete an 'opt out' form before 30th June 2010 and return it to Madeley Practice. The opt out form can be downloaded as a PDF file either from www.nhscarerecords.nhs or from the Home page of this website.
